Membership and Seminar Registration

Membership Fees: Dues are free for the first year for all FL DC's and $99. for each following year for Florida DC's and free for all out of state DC's.  To register for any seminar you need to fill out the form below and make a note in the comment box as to which seminar you want to attend. August 16-18, 2013 Seminar Fees: No charge:  Doctors licensed under 2 years                     CA's                     Retired DC's that do not need credits Reduced Fees:  Out of state DC's  $50. Paid Up Members: $75 Non-Paid-Up Members: $175 May 18, 2013 PIP Seminar Fees: Paid up member $65 Non-Paid $100 Ft. Lauderdale CE Seminar. Registration will not start until after the August CE Seminar. If you want to pay by check: FCPA 5721 Padgett Cir. Orlando, Fl. 32839 Upon completion of the hours you sit for you will receive CE Voucher form .  It is to be sent to the UB Post Grad Dept with a $20. credentialing fee
